Some state‐based newborn screening programs in the United States already use sequencing technology, as a secondary screening test for individual conditions rather than as a broad screening tool. Newborn screening programs sequence an individual gene, such as the cystic fibrosis transmembrane conductance regulator, which causes cystic fibrosis, after an initial biochemical test suggests that a baby might have a condition related to that gene. The experiences of state public health departments with individual‐gene sequencing illustrate both the usefulness of the technology (...) and its complexities. Here I discuss how newborn screening programs investigate cystic fibrosis and, as another example, adrenoleukodystrophy through individual gene sequencing. (shrink)

The goal of this paper is two-fold: first, to emphasize causality in disease ontology and knowledge representation, presenting a general and cursory discussion of causality and causal chains; and second, to clarify and develop the River Flow Model of Diseases (RFM). The RFM is an ontological account of disease, representing the causal structure of pathology. It applies general knowledge of causality using the concept of causal chains. The river analogy of disease is explained, formal descriptions are offered, and the RFM (...) disease definition is refined by describing causal chains in terms of causal relations. The definition is updated to coincide with the actual RFM classes found in its upper-level ontology, YAMATO, which brings to light some challenges in developing both YAMATO and the RFM. The RFM is also discussed in relation to another ontological account of disease. Strategies are offered toward interoperability between these theories. (shrink)

Who can divine the intentions of the human heart, the motives that guide behavior? Some of the reasons for our actions lie on the surface of consciousness, whereas others are more deeply embedded in the recesses of the mind. Recovering motives and intentions is a principal job of the historian. For without some attribution of mental attitudes, actions cannot be characterized and decisions assessed. Since Sydney Shoemaker published his seminal article ‘Self-Reference and Self-Awareness’ in 1968, the notion of ‘Immunity to Error through Misidentification’ has received much attention. It crops up in discussions of personal identity, indexical thought and introspection, and has been used to interpret remarks made by philosophers from Wittgenstein to William James. The precise significance of IEM is often unspecified in these discussions, however. It is unclear, for example, whether it constitutes an important status of judgments, whether it explains an important (...) characteristic of judgments, or whether it merely marks an important characteristic of judgments. Nevertheless, reference to IEM abounds, making this obscure notion seem all the more significant. (shrink)

Thomas Henry Huxley recalled that after he had read Darwin’s Origin of Species, he had exclaimed to himself: “How extremely stupid not to have thought of that!” (Huxley,1900, 1: 183). It is a famous but puzzling remark. In his contribution to Francis Darwin’s Life and Letters of Charles Darwin, Huxley rehearsed the history of his engagement with the idea of transmutation of species. He mentioned the views of Robert Grant, an advocate of Lamarck, and Robert Chambers, who anonymously (...) published Vestiges of the Natural History of Creation (1844), which advanced a crude idea of transmutation. He also recounted his rejection of Agassiz’s belief that species were progressively replaced by the divine hand. He neglected altogether his friend Herbert Spencer’s early Lamarckian ideas about species development, which were also part of the long history of his encounters with the theory of descent. None of these sources moved him to adopt any version of the transmutation hypothesis. (shrink)

Reports an error in the original article by R. J. Smith . On pages 160, 161, 166, and 167 the subject to object relationship was reported at "S/O". The corrected representation is "S⇔O". The value-fact or subject-object split recently defended by H. H. Kendler as necessary for a scientific psychology to establish facts, was rejected by Gestalt psychology as reducing the person to object status. The Gestalt solution correlating principles of perceptual organization with corresponding features of the object world has (...) however answered poorly to the vast cultural differences found in values. Communal/dialectical psychology in agreement with a postmodern worldview, treats facts as intrinsically value-laden social constructions mediated by a society's particular social relations Examples of fact ambiguity are illustrated, and S⇔O is recommended as ontologically preferable for psychology as social science and for turn-of-the-millenium psycho-ecology. 2012 APA, all rights reserved). Ivan Illich, philosopher, historian, priest and social commentator died in Bremen, Germany on December 2, 2002. Illich was noted for his critique of the Church, education and medicine but his concepts dealt with more fundamental issues. This article reveals aspects of Illich, the man, and explores his ideas as they apply to the meaning of medicine and, in particular, the role of health care in contemporary society.

In the last number of years scholars have discovered many new “parallels”2 to Francis of Assisi’s Admonitions.3 In this article I will provide more new parallels that I have uncovered not only in ecclesiastical contexts, but also in non-ecclesiastical ones.4 While almost all students of Francis’ Admonitions are acquainted with the general ecclesiastical contexts, most are unfamiliar with the non-ecclesiastical contexts evidenced by Cato’s Distichs, Daniel of Beccles’ Urbanus Magnus, Egbert of Liège’s The Well-Laden Ship, the Facetus, and a fourteen-volume (...) collection of medieval proverbs. From these parallels I will argue that the Admonitions of Francis of Assisi belong to the literary genre of Conduct Literature... (shrink)
